If you have windows 7, sometime it can interfere with that. I guess it has problems with 4 cores as well, so you could always google troubleshoot options for stuff like that, but specifically, I can't help you with more details.

I tried all the FalloutFixes/Nexus forums and it wasnt working for me I was getting really frustrated then I found this. Its so simple and it fixed it, completely no strings attached. it made me laugh at all the trouble I went to like what Kevin Lou did with no results. Here:

the d3d9.dll file fixed everything and it was really easy. Good luck Fallout 3 is totally worth it so fight for it!!! ;)

Fallout New Vegas is a much newer game, and is therefore built to run on Windows 7. Fallout 3 has issues with it since it was made for a slightly older OS.

Have you tried opening up the fallout.ini file in: My Documents\My Games\Fallout3
got to the line
bUseThreadedAI=0
and change it to
bUseThreadedAI=1
then add another line after it and insert
iNumHWThreads=2
This limits the game to use only 2 cores in a quad core system and prevent the engine bug from causing the game to freeze.
